Instructions

1. Enter your contact information, including your First Name, Last Name, E-mail Address, and Telephone Number.
2. Enter your position title. If unemployed, leave blank.
3. Select, 'Employee', and 'Continue with Questionnaire'.
4. Proceed to 'Part A' of the questionnaire and follow the instructions.
5. Once you have completed 'Part A', proceed to 'Part B' and complete the questionnaire.  NOTE: If you are unemployed answer as though you are still working at your most recent position.
